Monitoring Sign Up question

I would like to sign up for Envisilink Monitoring. Our local sheriffs department requires an "Alarm User Registration" to be submitted. The form says our alarm must be registered BEFORE activation. The form requires the name, phone number and alarm company license number for the alarm company. Am I able to get this information so I can submit this form to the sheriffs department.
